The Ohio Department of Health confirmed the Pine Kirk Care Center and its sister facility in Utica, the Utica Care Center, have voluntarily closed.
The Pine Kirk Care Center was the site of a deadly shooting in 2017.
Thomas Hartless killed Kirkersville Police Chief Steven Eric DiSario, Marlina Medrano and one of her co-workers Cindy Krantz.
Nine staff members hid inside the facility until SWAT team members safely rescued them.
The Pine Kirk Care Center reopened in May 2018.
The health department confirmed all residents in the two centers have been moved to new facilities and it was given proper notification of the closures.
